At The Humanity Project, we believe everyone deserves the dignity of stable housing, access to quality health care, and the support needed to thrive. We provide no-cost Enhanced Care Management (ECM) and Community Supports (CS) to eligible Medi-Cal members throughout Northern California.

Our team of experienced health and housing navigators works one-on-one with individuals and families facing housing instability, complex health needs, behavioral health challenges, and other barriers to care. We take the time to understand each person's unique situation and develop a personalized plan that connects them with health care, housing resources, benefits, transportation, and community services.
