Enhanced Care Management (ECM)
ECM is a person-centered care coordination service available to eligible Partnership HealthPlan members with complex medical, behavioral health, or social needs. Each participant is paired with a dedicated navigator who helps coordinate services, remove barriers to care, and support long-term stability and wellness.

Who Qualifies for ECM?
ECM may be available to individuals experiencing housing instability, complex health conditions, behavioral health challenges, frequent hospitalizations, or other circumstances that make navigating services difficult.
Community Supports (CS)
Community Supports (CS) are housing-related services available to eligible Partnership HealthPlan members through CalAIM. These services help individuals obtain, maintain, and sustain safe housing while reducing barriers that impact health and well-being.
Our team provides personalized support and guidance to help members build long-term stability and independence.

We help members identify, secure, and move into stable housing.
Services may include:

Move-in planning and coordination
② Housing Deposits & Move-In Assistance
Financial assistance may be available to help eligible members overcome barriers to obtaining housing.

Once housed, we help members maintain housing stability and prevent future housing crises.
