How CalAIM Helps Us Support Members
CalAIM, short for California Advancing and Innovating Medi-Cal, is a statewide Medi-Cal initiative that allows eligible members to receive additional support beyond traditional medical care.
Through CalAIM, Partnership HealthPlan members may qualify for services such as Enhanced Care Management and Community Supports. These services can help with care coordination, housing navigation, transportation support, connection to community resources, and other needs that affect health and stability.
The Humanity Project provides services through CalAIM to help eligible Medi-Cal members get connected, supported, and guided through the next steps.
